Only one player on offense (the quarterback) and one player on defense (typically a linebacker) are allowed to have radio receivers in their helmets. Kyle Shanahan of the San Francisco 49ers has been one of the most innovative users of the wristband system. Shanahan's offense uses complex formations with multiple pre-snap motions, which produce long verbal play calls. By using wristband numbers, he can call these plays in less time, allowing the offense to operate at a faster pace and prevent the defense from substituting personnel.
The wristband is also a security measure. If the radio fails (which happens occasionally due to equipment malfunction), the quarterback can call plays from the wristband without coach communication. The NFL's official rules require that each team have a backup communication plan for radio failure, and the wristband serves this purpose.
The numbered play system is vulnerable to the same attack as any code book: if the opposing team obtains the wristband, they can decode every play call. Teams guard their wristbands closely and change the numbering each week. In 2012, the NFL fined the New York Jets for attempting to steal the New England Patriots' play signals from the sideline, a violation of the league's competitive integrity rules.
When the quarterback reaches the line of scrimmage and sees the defensive alignment, they may need to change the play. This is done through an audible: a verbal code shouted loud enough for teammates to hear. The audible is the least secure communication channel in the NFL because the opposing team hears it too.
Audibles use coded words that map to specific play changes. A quarterback might shout "Omaha" to signal a change to a specific run play, or "kill kill" to switch from a pass to a run. Peyton Manning's "Omaha" audible became famous during his tenure with the Denver Broncos. The word itself was arbitrary. Its meaning was defined by the team's weekly game plan and changed from game to game.
Hot routes are a specific type of audible used when the quarterback detects a blitz. The quarterback calls a "hot" signal that tells receivers to change their routes to quick short passes that can be thrown before the blitz arrives. The hot route system is a prearranged response to a specific defensive look, and it must be communicated in seconds.
The security of audibles depends on the opposing team not knowing the code words. Since the words change weekly, a team that has not observed the opponent's current game plan cannot decode them in real time. However, if a team has film of previous games, they can build a frequency table of audible words and correlate them with the resulting plays, similar to how codebreakers analyze intercepted messages. Defense faces a different communication problem. The defensive coordinator cannot use a radio to communicate with all 11 players. Defensive signal-calling uses large hand gestures visible from across the field. A signal-caller on the sideline (often a backup coach or player) flashes a sequence of signs that encode the defensive play: front, coverage, blitz package, and adjustments. The signs are arbitrary gestures (a hand to the helmet, a clap, a sweep of the arm) that map to specific calls in the defensive playbook.
The vulnerability is obvious: the opposing offense can see the signals. Teams counter this by using decoy signals. Only one of several visible signal-callers is giving the real signs. The others are decoys. The offense must figure out which signal-caller is live, which is a problem of identifying the correct channel among noise.
